SEARCH VIRGINIA’S DBHDS-LICENSED PROVIDERS:

  • LICENSED PROVIDER LOCATION SEARCH to find more information about providers that have been licensed or are in the process of being licensed by DBHDS. Search instructions: Click on the drop-down box arrows to refine your selections.  Click on the “+” (plus sign) to display more detailed information.
  • PROVIDER INSPECTION/INVESTIGATION REPORTS SEARCH to find information about providers in Virginia who are licensed by DBHDS. Search instructions: Click on the Provider drop-down box (gray arrow), scroll down and click on the desired provider name.  Next, click on the Services drop down box (blue arrow), scroll down and click on the desired provider service-location. Finally, select a Report Type to view any inspection or investigation report completed after January 1, 2012.

ADDITIONAL INFORMATION FOR INTELLECTUAL DISABILITY (ID) LICENSED PROVIDERS:

  • Developmental Disability Licensed Providers with Conditional Licenses – Providers and services that have been licensed for the first time. A conditional license is issued to a new provider for services that demonstrates compliance with administrative and policy regulations but has not demonstrated compliance with all the regulations. This listing will be updated on a monthly basis (Updated September 5, 2021).
  • Developmental Disability Licensed Providers with Provisional Licenses- Providers and services that have a provisional license. A provisional license may be issued to a provider for a service that has demonstrated an inability to maintain compliance with regulations, has violations of human rights or licensing regulations that pose a threat to the health or safety of individuals being served, has multiple violations of human rights or licensing regulations, or has failed to comply with a previous corrective action plan. This listing will be updated on a monthly basis (Updated September 5, 2021).
  • DD Group Homes with Four or Fewer Beds – Providers that are licensed to group homes for up to four individuals. A Group home residential service” means a congregate service providing 24-hour supervision in a community-based home. Services include supervision, supports, counseling, and training in activities of daily living for individuals whose individualized services plan identifies the need for the specific types of services available in this setting. This listing will be updated on a monthly basis

  • DD Group Homes with Five or More Beds  – Providers that are licensed to group homes for five or more individuals. A Group home residential service” means a congregate service providing 24-hour supervision in a community-based home.   • ICF/IID – Providers that are licensed as Intermediate Care Facilities for Individuals with Intellectual Disability (ICF/IID). An Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF/IID)” means a residential facility in which care is provided to individuals who have intellectual disability or a developmental disability who need more intensive training and supervision than may be available in an assisted living facility or group home.
